Elevated
liver enzymes can indeed be a sign of liver damage/issues but
very often it comes as the result of medications and even concentrated
amounts of herbs. Your mention of Duloxetine immediately comes
to mind. (But as Kate says, don't take this as gospel...)

couple of years ago I got into a super-heroic mindset and started
taking PHENOMENAL quantities of liquid Olive Leaf Extract and
I was using it to detox. Well, I got some blood chemistries done
and sure enough...there were elevations in my enzymes. I freaked
so hard that I got a liver sonogram and everything was A-Okay.
But what a scare...yeah, that will show up on a blood test.
